Use this form to log hours you volunteered off-site as a committee member, board member, or as a volunteer at a special event. It may also be used to log hours not reported in the volunteer log-in computer. Thank you for your volunteer service.

Please submit this form upon completion of your volunteer time. If you worked in more than one category (i.e. Program and Office), complete another form to enter the time in and time out for each role.

Enter Volunteer ID (find yours in the binder near the computer.) The format should be V-####. Be sure to enter both the V and the "-".
